What are the causes of Femoral Neck Thinning?

The three main causes of neck thinning are
1) a vascular insult to the femoral neck such as failure to use vent or stripping soft tissue of the femoral neck.
2) excess wear debris such as from edge wear of the cup. 
3) The 3rd cause of neck thinning is deep infection although I hope this would have already been excluded in the work-up.
